1. Who We Are and What This Policy Covers

Oracle Multimedia Enterprises LLC, doing business as Priority Home Monitor (collectively, "PHM," "we," "us," or "our"), provides the Priority Home Monitor mobile application, its supporting websites, and related services (the "Service"). This policy explains how we collect, use, disclose, retain, and protect personal information through the Service.

PHM is currently an invitation-only remote health-monitoring pilot. Accounts are created by an authorized program administrator. The pilot is intended to use synthetic, non-identifying information only. Do not submit real patient information or protected health information unless PHM has expressly authorized your organization to do so under an appropriate written agreement.

PHM supports monitoring and communication. It is not an emergency service, does not replace professional medical judgment, and does not diagnose or treat any condition. Call 911 or your local emergency number in an emergency.

2. Information We Collect

Account and contact information

Username, name, email address, phone number, city and state, account status, care-team relationships, language, and accessibility preferences provided by you or an authorized account administrator.

Health and fitness information

Vital-sign readings, device measurements, symptoms, activity or step information, meals and nutrition results, reports, care-plan information, and other health-related information submitted through the Service.

User content and communications

Secure messages, support requests, questions submitted to PHM AI, meal descriptions, product barcodes, and meal photos that you deliberately choose to analyze.

Identifiers, device, security, and usage information

User and account identifiers; app version; device and operating-system information; IP address; authentication events; consent, security, and audit logs; feature-use events; and error or performance diagnostics needed to operate and protect the Service.

We collect information directly from you, from an authorized provider or account administrator, from a connected monitoring device you choose to use, and automatically when the app communicates with the Service. PHM does not collect precise or coarse location, contacts, payment-card information, or advertising identifiers through the current app version.

3. How We Use Information

We use personal information to create and authenticate accounts; deliver requested monitoring, communication, nutrition, reporting, and care-plan features; connect supported devices; respond to support requests; secure and troubleshoot the Service; maintain consent and audit records; comply with law; and improve reliability and safety.

We do not sell personal information, use health information for advertising, share personal information for cross-context behavioral advertising, or track people across other companies' apps or websites.

4. When We Disclose Information

We may disclose information to an authorized care team or program administrator, at your direction, or to vendors that help us provide the Service. We limit vendor access to the information reasonably needed for their assigned function and require appropriate confidentiality, security, and data-use protections.

  • Amazon Web Services (AWS)hosts PHM application services, encrypted databases and files, backups, logs, and security controls in the United States.
  • Groqmay process a question and the minimum relevant context when you deliberately use PHM AI and that feature is enabled. PHM AI is informational and is not a diagnostic or emergency service.
  • Google Geminimay receive a sanitized copy of a meal photo only after separate consent. Before transmission, PHM removes image metadata, resizes and re-encodes the image, and omits PHM account identifiers. PHM retains the derived nutrition result rather than the submitted photo.
  • USDA FoodData Central and Open Food Factsmay receive food-search terms or product barcodes so PHM can retrieve nutrition information.
  • Appleprocesses App Store distribution, device permissions, and platform diagnostics under Apple's terms and your device settings.

We may also disclose information when reasonably necessary to comply with law or legal process, protect a person's safety, investigate fraud or security incidents, enforce our terms, or complete a merger, financing, acquisition, or sale of assets. If ownership changes, personal information remains subject to this policy unless legally permitted notice and consent requirements are satisfied.

5. Camera, Bluetooth, Motion, and Device Permissions

PHM requests a device permission only when you use a related feature. Bluetooth connects supported monitoring devices. Motion access supports activity features. Camera or photo-library access supports a feature you deliberately start, such as meal-photo analysis. You can deny or revoke these permissions in iOS Settings; doing so disables only the related feature.

6. Consent and Your Privacy Choices

  • Review or correct profile information in the app or through your authorized program administrator.
  • Grant or withdraw meal-photo analysis consent in the food feature. Withdrawal stops future transmissions but does not undo processing completed before withdrawal.
  • Manage camera, Bluetooth, motion, notification, and photo permissions in iOS Settings.
  • Request access, correction, a portable copy, or deletion by using available in-app tools or contacting us.
  • Depending on where you live, you may have additional rights concerning personal information, including rights to confirm processing, appeal a denied request, or withdraw consent to processing sensitive data. We will verify requests and respond as required by applicable law. We will not discriminate against you for exercising an applicable privacy right.

7. Retention and Account Deletion

We retain account and application data while the account is active and as reasonably needed to provide the Service, protect users, resolve disputes, enforce agreements, and meet legal obligations. PHM's cloud application and security logs are configured for 90 days, and routine database backups are configured for 35 days, unless an incident, safety need, or legal obligation requires a longer hold.

You may start account deletion from Profile → Privacy & Consent → Delete Account. After successful reauthentication, PHM revokes active sessions and disables portal access while the request receives a retention review. Information that is not required for a permitted legal, security, audit, billing, or healthcare-record purpose will be deleted or de-identified. Information retained for a permitted purpose remains protected and is not used for unrelated purposes. See our Account Deletion page for details.

8. Security and Breach Notification

PHM uses safeguards designed to protect information, including encrypted network connections and cloud storage, role-based access controls, short-lived access tokens, refresh-token rotation and revocation, login throttling, audit logging, private application and database networks, and monitored backups.

No system is completely secure. If a breach occurs, we will investigate and provide notices required by applicable law, including applicable health-information breach-notification rules.

9. U.S. Health Privacy

Whether the Health Insurance Portability and Accountability Act ("HIPAA") applies depends on how PHM is offered and the relationships among PHM, healthcare organizations, and users. This policy is not a Notice of Privacy Practices under HIPAA. Where PHM processes protected health information for a HIPAA-regulated organization under a business associate agreement, that agreement and the organization's Notice of Privacy Practices may also govern. Other health-privacy and breach-notification laws may apply even when HIPAA does not.

10. Children

The current pilot is not directed to children under 18, and an account must not be provisioned for a minor without documented authorization and an approved workflow. If you believe a child's information was submitted improperly, contact us so we can investigate and take appropriate action.

11. International Use

PHM's current pilot is intended for users in the United States. Information is processed and stored in the United States. Do not use the pilot outside the United States unless PHM has expressly authorized that use and provided any additional notices required for your location.

12. Changes to This Policy

We may update this policy as the Service or law changes. We will post the revised policy, update the effective date, and provide any additional notice or obtain consent when required. If a change affects consent-based external processing, PHM may require acceptance of a new consent version before that processing continues.
